PARENT/GUARDIAN PERMISSION SLIP, MEDICAL CONSENT & LIABILITY WAIVER
Youth Group VBS Mission Trip To Kremmling β July 26, 2026
1. Permission to Participate
I give permission for my child, your child, to participate in this mission trip and all related activities, including travel, lodging, worship gatherings, service projects, group meals, recreational activities, ministry assignments, and supervised group events.
I understand that participation in this mission trip is voluntary and that my child is expected to follow all instructions, safety guidelines, schedules, and behavioral expectations provided by Redeemer Baptist Church, trip leaders, host organizations, transportation providers, lodging facilities, and ministry partners.
2. Acknowledgment of Risk
I understand that mission trips may involve risks, including but not limited to travel delays, vehicle travel, physical activity, unfamiliar environments, weather conditions, illness, injury, allergic reactions, emotional stress, interactions with the public, and other circumstances that may be outside the control of Redeemer Baptist Church.
I understand that while Redeemer Baptist Church will make reasonable efforts to provide supervision, safety procedures, and responsible leadership, no activity can be guaranteed to be free from all risk.
I knowingly give permission for my child to participate with an understanding of these risks.
3. Transportation Permission
I give permission for my child to travel with Redeemer Baptist Church, its staff, adult volunteers, approved drivers, ministry partners, or transportation providers during the mission trip. This may include travel by church vehicle, personal vehicle driven by an approved adult, rental vehicle, bus, airplane, public transportation, shuttle, or other transportation necessary for the trip.
I understand that seat belts are required where available and that participants must follow all transportation safety instructions.
4. Medical Treatment Authorization
In the event of illness, injury, accident, allergic reaction, medical emergency, or other health concern, I authorize the adult leaders of Redeemer Baptist Church to seek and approve medical care for my child when I cannot be reached in a timely manner. This may include first aid, emergency medical treatment, urgent care, hospital care, diagnostic testing, medication administration, ambulance transportation, or other treatment deemed necessary by medical professionals.
I understand that I remain responsible for any medical expenses, insurance deductibles, co-pays, prescriptions, transportation costs, or other expenses related to medical care provided to my child.
5. Medical Information and Medications
I agree to provide accurate and complete medical information for my child, including allergies, medications, medical conditions, dietary restrictions, physical limitations, and emotional or behavioral concerns.
All prescription medications must be provided in their original container with the participant's name, dosage, and instructions clearly listed. I give permission for designated trip leaders to assist with medication according to the instructions I provide.
6. Behavior Expectations
My child is expected to behave respectfully, safely, and responsibly throughout the trip β following the direction of adult leaders, respecting other participants, honoring the host community, remaining with the group, participating in scheduled activities, avoiding unsafe behavior, and representing Redeemer Baptist Church in a Christ-honoring way.
Prohibited behavior includes: leaving the group without permission; possession or use of alcohol, tobacco, vaping products, illegal drugs, weapons, or inappropriate materials; bullying, harassment, threats, fighting, sexual misconduct, or abusive language; repeated disrespect toward leaders or participants; unsafe behavior that endangers anyone; and violation of trip, lodging, host-site, or transportation rules.
If my child's behavior becomes unsafe, disruptive, illegal, or contrary to trip expectations, I understand that Redeemer Baptist Church may contact me and may require my child to return home early. I may be responsible for transportation costs, change fees, lodging costs, supervision costs, or other expenses related to early return.
7. Personal Belongings
My child is responsible for their personal belongings, including luggage, clothing, electronics, money, medications, and other items. Redeemer Baptist Church is not responsible for lost, stolen, or damaged personal property.
8. Release of Liability
To the fullest extent permitted by law, I release and hold harmless Redeemer Baptist Church, its pastors, staff, officers, directors, elders, deacons, employees, volunteers, trip leaders, approved drivers, ministry partners, host organizations, and representatives from claims, demands, causes of action, liability, damages, losses, or expenses arising out of or related to my child's participation in this mission trip, except where prohibited by law. This does not excuse intentional misconduct, gross negligence, or any liability that cannot legally be waived.
9. Certification
By submitting this registration, I certify that I am the parent or legal guardian of the participant; I have read this permission slip, medical consent, and liability waiver; I understand the nature of the mission trip and give permission for my child to participate; and I authorize emergency medical care if needed.
